THE BERKLEY CHOP SHOP L.L.C. ASSUMPTION OF RISK & WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19
The novel Coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies have proposed recommendations for a safe reopening. The Governor of the State of Michigan, Gretchen Whitmer, has allowed barbershops as of June 15, 2020 to reopen to the public.
The BERKLEY CHOP SHOP L.L.C. (CHOP SHOP) has put in place preventative measures to reduce the spread of COVID-19; however, the CHOP SHOP cannot guarantee that you, your spouse, your child(ren) or other family members or others with whom you have contact will not become infected with COVID-19.
1. By signing this ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I, my spouse, my child(ren), or others with whom I/We have contact may be exposed to or infected by COVID-19 by the services performed at the CHOP SHOP. Some of these risks that I undertake may cause sickness or further complications, known or unknown. The CHOP SHOP is taking all necessary precautions applicable to barber shops, recommended by the State of Michigan, as outlined in the posted signs.
2. I voluntarily agree to assume all of the foregoing risks, both known and unknown, and accept sole responsibility for any sickness or further complications to myself, my spouse, my child(ren) or others due to my attendance at the CHOP SHOP for services rendered. I now release, covenant not to sue, discharge, and hold harmless the CHOP SHOP, its employees, agents and representatives, independent contractors and other participants, for any and all claims, including all liabilities, actions, damages, costs or expenses of any kind whatsoever arising out of or relating to the services rendered by the CHOP SHOP.
